What is PresbyMAX

After 40, the lens progressively loses its ability to focus at close range (presbyopia). PresbyMAX solves this with the laser: it sculpts the cornea with a bi-aspheric multizonal ablation profile — central zone for near, pericentral zone for distance, transition zone for intermediate.

The most prescribed variant is μ-Monovision: faster neural adaptation compared to other variants, maximum comfort in daily life, excellent spatial vision. The choice of variant is personalised based on ocular dominance.

The AMARIS 1050RS laser sculpts the cornea without touching the eye. No flap. No incision. Presbyond and Supracor require a corneal flap. IOL requires an intraocular incision. PresbyMAX does not.
How to choose the best technique
All techniques are a compromise.
PresbyMAX is the only one without contact.

After 40, the lens loses elasticity and the accommodation mechanism — the eye's ability to focus at different distances — no longer works. No technique restores it: all work around this limitation, compensating for it differently. PresbyMAX, Presbyond, Supracor e IOL are all valid approaches, with different indications for age, anatomy and expectations — and all involve a visual compromise. The difference with PresbyMAX is just one: it is the only one completely without contact with the eye — no flap, no incision.

Safety and flexibility

PresbyMAX
is reversible.

It is not an irreversible decision. Schwind prevede three modification or reversal options — available from 6 months after the initial procedure, once healing is complete.

Option 1
PresbyMAX repetition
If presbyopia progresses or reading quality is no longer sufficient but distance vision satisfies, the PresbyMAX treatment can be repeated to increase the multifocal effect.
Option 2
Aberration-Free treatment
If near vision satisfies but one wishes to improve distance, an Aberration-Free treatment on the same optical zone optimises the distance correction.
Option 3
Reversal PresbyMAX
If the patient does not accept the multifocal compromise, è a complete reversal treatment with refraction correction is possible — returning to a vision optimised for distance.

No. The procedure is painless thanks to anaesthetic drops. In the first 48–72 hours, burning, tearing and photosensitivity may appear — normal symptoms of epithelial healing, manageable with prescribed eye drops.

Yes. The bi-aspheric multizonal profile optimises each corneal zone for a different distance — the brain integrates the signals naturally. The neural adaptation period is 4–8 weeks. With μ-Monovision — the most prescribed variant — recovery is particularly rapid. In rare cases, neural adaptation is slower and visual quality perfects completely after a few months.

Yes. The ablation profile is permanent but the treatment is reversible. Schwind provides three options: repetition of PresbyMAX if presbyopia progresses, Aberration-Free treatment to improve distance, or complete reversal (Reversal) if the patient does not accept the multifocal compromise. All re-treatments are available from 6 months after the procedure.

PresbyMAX è a corneal laser treatment: no intraocular incision. Multifocal IOL replaces the lens with an artificial one — typically indicated after 60 or in case of advanced cataract. For patients between 40 and 60 with a transparent lens, PresbyMAX is the preferable and reversible approach.

In the first 4–8 weeks, halos and night light reflections may appear — effect of neural adaptation. In the vast majority of patients they subside within 3–6 months. Night driving in the first weeks should be agreed with the surgeon.
